How Effective is Vasectomy Reversal Surgery?

A vasectomy is a procedure to seal and cut off the tubes, known medically as the vas deferens, that carry sperm from the testicles to the penis. Vasectomy is a widely used form of permanent contraception, chosen by more than 15 per cent of UK men when they decide that do not want to have any more children. However, unanticipated changes in family circumstances, such as divorce and a new partner, mean approximately one per cent of men who have had a vasectomy feel they would like to have more children and therefore consider vasectomy reversal.

Vasectomy reversal surgery is performed under general anaesthetic, usually as a day case. A cut is made in the middle of the scrotal sac and the cut ends of the vas (the tubes) are identified and joined together with stitches. It takes about an hour. Most men are on their feet a couple of hours later and able to return home later on that same day.

Reversal can be attempted in almost all men who have had a vasectomy but success rates vary significantly. There are two different assessments of the effectiveness of a vasectomy reversal: whether any sperm is produced (patency rate) and whether the man is able to conceive a child with his partner (pregnancy rate). The patency rate depends on male factors alone, whereas the pregnancy rate is dependent upon both male and female factors.

The following factors influence the effectiveness of a vasectomy reversal:

Length of time since vasectomy
If a long time has passed since the vasectomy, sperm production may have stopped or significantly lowered. But if the reversal is undertaken within ten years of the vasectomy, the sperm production rate should be around 85 per cent.

The effect of original vasectomy reversal surgery
At the time of the vasectomy, if a large amount of the vas (tube) has been removed, it may not be possible to reconnect the vas ends together.

Female partner factors
The first two factors above relate to the patency rate, which means the production of sperm. However pregnancy is of course dependent upon both male and female fertility. The most important question is that of the female partner's age. Age is less significant for men than women; if the woman is aged less than 35 and the vasectomy was performed less than 10 years earlier, the pregnancy rate following vasectomy reversal is roughly 50 per cent. The pregnancy rate after vasectomy reversal falls to less than 20 per cent if the woman is in her forties and the vasectomy was performed more than a decade earlier.

Comparing vasectomy reversal rates reported by different clinics
When comparing results from different clinics and surgeons, it is important to ensure that the measures used by each clinic are the same, in order to gain a fair and accurate comparison. The sperm production rate will always be higher than the pregnancy rate because it only takes male factors into account. Also, some clinics only publish their results after omitting certain groups of people from their study group - for example men whose partners are over 35.

What should I expect after vasectomy reversal surgery?
After vasectomy reversal surgery, there may be some minor swelling or bruising which can be managed with standard pain killers. It is important to take it easy for a week, but avoid any sports or strenuous activities for two weeks. Sexual intercourse can take place a week after surgery provided you are careful. It takes several weeks after a reversal for sperm to build up and at 12 weeks after surgery, a sperm test will confirm whether or not the reversal has been successful. If there is some sperm production, but sperm count is rather low and of poor quality, your chances of conceiving will be lower. The interaction of male and female factors is important - if sperm count is low, but partner is under 35 with no fertility problems, your chance of a pregnancy will be higher than if there are fertility problems on both sides. Occasionally, women become pregnant very quickly after a reversal. If there is no pregnancy after a year of the reversal, it would be advisable to seek further tests and advice.
